Free vet clinic walk-in check-in sheet
A check-in sheet for a walk-in vet clinic: the owner, the pet name, dog, cat or other, the reason for the visit, time in, and a box for an urgent visit. Add your clinic name, then print it or download the PDF. No email or sign-up.
A link where customers can join from their phone. The sheet prints it as a QR code with "Skip the clipboard". Leave it empty for paper only.
To save your version as a PDF, press Print and choose "Save as PDF". US Letter, one page.
What the columns are for
Species tells the front desk which room, which tech and which carrier or leash the visit needs. Cats and dogs in the same waiting room are easier to separate when the sheet already says which is which.
A short reason lets the team sort a vaccine or nail trim from a limp or a vomiting pet. Keep it to a few words; the clinical details belong with the vet, not on a sheet others can read.
The "Urgent" box is the owner flagging that they think the pet needs to be seen soon. It is a prompt for the team, who decide the order. It is not a medical judgment.
Time in lets the front desk see how long each pet has waited, so a quiet owner is not forgotten behind a loud one.
When the clipboard stops working
Paper works while the lobby is small. It stops when people look at a full room and leave without signing, when customers wait for one person, or when someone asks "how long?" and nobody knows. A sheet records arrivals. It can't show a wait, and it can't tell anyone it's their turn.
NextUp Walk-in is the same list on a phone. Customers scan a code at the door, see their place and wait, and follow it live from anywhere (text alerts coming soon). A TV in the lobby shows who's next. It's free for one location, one team member and 50 check-ins a month; paid plans, with the lobby TV, start at $29 a month for the first location, with a 14-day free trial. More in paper, Google Forms or a real waitlist.
Common questions
Why is there no phone number column?
A sign-in sheet sits on the counter where every customer can read it. A column of names and phone numbers is a list anyone can photograph. If you want to text people when it is their turn, take the number somewhere private, such as a phone Check-in Page that only the shop can see.
Can a walk-in vet clinic run on a paper sign-in sheet?
Yes, for a front desk that sees a steady trickle of walk-ins. The sheet gives the team the order, the species and the reason at a glance. Once the line gets longer than a clipboard, a phone Check-in Page lets owners wait in the car.
What does the urgent box mean?
It is a mark the owner makes when they think the pet needs to be seen soon. The staff decide who goes first. The sheet only makes sure the team sees that the owner is worried.
How much should owners write about the reason for the visit?
A few words, such as "shots", "limping" or "nails". The sheet is on the counter where other clients can read it, so full symptoms belong with the vet in the exam room.
Where do vaccination records and patient notes go?
Keep them in your patient records. The sheet only lists who is waiting and why.
Is this sign-in sheet free?
Yes. Print as many as you like, with no account and no email.
