## What the columns are for

The "Seeing" column keeps the provider in view. In a clinic with more than one chiropractor, a return patient who asks for their own doctor should not be called by someone else.

New or return sets the time. A first visit means paperwork and a longer exam, while a return visit is often quicker. The desk can then tell the next patient an honest wait.

The "Forms" box records that the intake or consent forms are done today, so a new patient does not reach the room without them. Keep the forms themselves in the chart, not on the sheet.

There is no reason-for-visit column. A sheet on the counter is read by everyone in the waiting room, so symptoms and conditions belong in the exam room.

## When the clipboard stops working

Paper works while the lobby is small. It stops when people look at a full room and leave without signing, when customers wait for one person, or when someone asks "how long?" and nobody knows. A sheet records arrivals. It can't show a wait, and it can't tell anyone it's their turn.

NextUp Walk-in is the same list on a phone. Customers scan a code at the door, see their place and wait, and follow it live from anywhere (text alerts coming soon). A TV in the lobby shows who's next. It's free for one location, one team member and 50 check-ins a month; paid plans, with the lobby TV, start at $29 a month for the first location, with a 14-day free trial. ## Common questions

Why is there no phone number column?

A sign-in sheet sits on the counter where every customer can read it. A column of names and phone numbers is a list anyone can photograph. If you want to text people when it is their turn, take the number somewhere private, such as a phone Check-in Page that only the shop can see.

Why is there no column for symptoms or the reason for the visit?

The sheet sits where other patients can read it. A name next to a condition shows more than the patient meant to share, so the sheet carries only a first name, the provider and new or return. The reason goes on the intake form.

Can a walk-in chiropractic clinic use a paper sign-in sheet?

Yes. It shows the order, the provider and whether each person is new, which is all a front desk needs to call patients fairly. Keep clinical notes in the chart.

Should patients write a full name?

A first name is enough for calling people in order. If two patients share a first name, add a last initial at the desk.

How do I handle a patient with an appointment?

Keep scheduled patients in your book. If an appointment holds a room at a set time, note that time on the sheet so the next walk-in hears an honest wait.

Is this sign-in sheet free?

Yes. Print as many as you like, with no account and no email.
