# Waiver (consent form)

A waiver is a signed form in which a customer acknowledges risks or gives consent before a service, commonly used in tattoo, piercing, spa and pet care.

Also called: consent form, release form, intake form

Some services carry risk or need informed consent, and a signed form is how a business documents it. A tattoo studio collects consent and age verification. A groomer may ask for vaccination details. A spa asks about allergies. The form is part of the visit as much as the chair.

On paper, forms are slow and easy to lose. Digital forms let a customer fill them in on their own phone before sitting down, and the signed copy is stored with their record. That also helps on repeat visits, since a returning client need not start over.

What a waiver actually does in law depends on the state and the service, so the wording is for you or your attorney to decide. The software can collect and store the form, but it cannot tell you what it should say.
