Waivers and intake forms are available on every plan. Can a client skip a form? Only an optional one. A required form has to be completed before the client can join the line or book online. Where does the signed copy go? On the client’s profile, with the signature and the time it was signed. Is this a medical record system? No. NextUp Walk-in runs intake and consent forms.
